EMERGENCY CONTACT INFORMATION
Assumption of risk:
I agree to indemnify and hold harmless Central College and any of its agents, employees, or representatives for any injury or loss suffered by me due to my
participation in the activity or activities specified above (hereinafter collectively referred to as “activity”). I hereby agree that I have been fully advised of
the nature and extent of the activity that may take place and represent to you that I am physically and mentally able to participate in the activity without special
accommodations or additional supervision.
I understand that the activity may present the risk of injury, or even death, to me, and I have been fully advised of those possibilities. I represent to you that I fully
assume the risk of any such injury or death, and I hold you, your agents, employees, and representatives harmless from any liability or death to me while engaged in this
activity that is caused or contributed to by my conduct or the conduct of any other participants. I further agree to indemnify and defend you, your agents, employees,
and representatives against any claim or liability asserted for any such injury or death.
I also hold you, your agents, employees, and representatives harmless from all liability to any other person or entity arising as a result of the conduct of myself or
the other participants in the activity and I agree to defend and indemnify you, your agents, employees, and representatives against any claim or liability arising as a
result of such conduct.
If I am not able to be consulted for any reason in the case of an emergency or necessity arising during the course of the activity or as a result of the activity,
I authorize you to contact the emergency contact person or persons listed above and to arrange for such medical and hospital treatment as you may deem to be advisable
for my health and well being.

Parent/Legal Guardian Consent: I agree to allow my child to participate in LEADership camp at Central College.

Misbehavior: I am aware that if my child needs to be removed from camp/clinic due to misbehavior, I will be responsible for his/her transportation home from Central College

Loss of Personally-Owned Property: My child shall be solely responsible for any and all damages or loss by theft or otherwise of personal property whether such property belongs to the student or to others.

In consideration of Central College, their trustees, agents, officers, volunteers, participants, employees and all other persons acting on their behalf for the
LEADership camp, as either participant or parent/guardian, I do hereby agree to release and discharge the LEADership camp, on behalf of myself, my children, my
parents, my heirs, assigns, personal representatives and estate as follows:
a. I agree to indemnify and hold harmless the Board of Trustees of Central College, Central College, the LEADership camp, and their directors, employees and agents
from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney’s fees brought as a result of taking part in the
LEADership camp, activities and to reimburse them for any such expenses incurred.
b. By signing below, I acknowledge that if anyone is hurt or if property is damaged during my participation in the activities, I may be found by a court of law to have
waived my right to maintain a lawsuit against the LEADership camp, on the basis of any claim from which I have released them herein.
Waiver Agreement

Photo Release (Optional)
I hereby grant Central College permission to use, reproduce, or distribute any photographs, digital images, films, videotapes, and/or sound recordings of me during my participation
in this Camp, for use in materials that may be created.
